Each of us brings years of experience in rehabilitation, but what really makes us different is our passion for helping older adults stay active and independent.

Natalie Tweed – Founder and Clinical Director

Natalie is the founder and Clinical Director of Home Grown Independence, bringing a wealth of expertise and a passionate commitment to supporting older adults in maintaining their independence and wellbeing.
Natalie qualified as a physiotherapist in 2010 and recently achieved a master’s with distinction in Advanced Clinical Practice in 2021. She has over 15 years of experience working within the NHS, specialising in community older adult rehabilitation, falls prevention, and urgent adult care.
Natalie founded Home Grown Independence to follow her vision of preventing disability, injury, and dependency through a proactive, holistic approach. She is dedicated to helping individuals build and maintain strength, balance, confidence, and physical fitness, ensuring they can live safely and independently in their own homes.
With her extensive clinical experience and passion for person-centred care, Natalie brings a unique combination of knowledge, skills, and holistic perspective to every assessment and treatment at Home Grown Independence.

Anna Gerrard – Senior Physiotherapist

Anna joined the Home Grown Independence team in March 2025 and has quickly become a highly valued member of our service. With over 18 years of experience working within the NHS, she has developed a deep expertise in older adult rehabilitation and is dedicated to promoting activity, health and independence in later life.
Her extensive background in elderly rehabilitation has given her wide-ranging experience across both hospital and community settings. Anna has worked closely with multidisciplinary teams including occupational therapists and speech and language therapists, ensuring holistic care for her patients.
She has specialist skills in:
• Stroke and neurological rehabilitation
• Falls prevention and balance training
• Orthopaedics and post-surgical recovery
• Pulmonary rehabilitation

Anna is committed to delivering excellent, person-centred care and takes pride in supporting people to achieve their goals.

Steph Gibson – Therapy Assistant Practitioner

Steph is a key member of the Home Grown Independence team, bringing an impressive 18 years of experience in the NHS across both intermediate care and community settings. She began her career in stroke rehabilitation, before supporting older adults with a wide range of mobility, physical, and cognitive health conditions.
Over the past 8 years, Steph has developed extensive expertise in:
• Orthopaedic rehabilitation
• Falls prevention and balance training
• Elderly rehabilitation
• Stroke and neurology recovery
Steph held a previous position as an accredited Moving and Handling facilitator, having trained NHS staff in safe and effective care practices.

Steph beings not only specialist knowledge but also a genuine enthusiasm for enabling older adults to remain mobile and engaged. Her passion, knowledge, and approachable nature make Steph a real asset to our team.
