What to Expect from your Follow-up Physiotherapy Sessions

 

After your initial assessment, your follow-up sessions are a chance to keep building on your progress.

These visits are relaxed, supportive, and focused on helping you feel more confident and independent in your own home.

At the start of each session, your physiotherapist will take a few minutes to chat and see how you’ve been since the last visit.

They might ask:

  • How you’ve been managing with your exercises or activities
  • Whether anything feels easier or more difficult
  • If you’ve had any new aches, pains, or falls
  • What has gone well and what you’d like to focus on today

This conversation helps tailor the session to your current needs and how you’re feeling that day.

Most follow-up sessions include guided practice of your exercises or everyday movements.

This might involve:

  • Reviewing how you’re doing your exercises to check technique and safety
  • Progressing or adapting them to make them a little more challenging
  • Practising tasks that matter to you – such as getting out of a chair, climbing stairs, or walking safely outdoors
  • Working on balance, coordination, and endurance

Your physiotherapist will always ensure activities are safe, suitable, and meaningful to you.

Together, you’ll look back at your goals you set during your first assessment.

Your physiotherapist will:

  • Review how far you’ve come
  • Celebrate improvements and milestones
  • Gently reassess key movements, balance, or strength if needed
  • Adjust your plan if your situation or confidence has changed

It’s normal for progress to vary – some days will feel better than others, and your physiotherapist will support you at you own pace.

Because sessions take place in your home, your physiotherapist can help you find practical ways to fit exercise into your routine- for example

  • Standing during TV advert breaks
  • Doing balance practice at the kitchen work top whilst kettle boiling
  • Using household items for strength work

They may also check your environment again if anything has change – such as furniture layout, new equipment, or seasonal hazards like wet paths or poor lighting.

Together, you’ll look back at your goals you set during your first assessment.

Your physiotherapist will:

  • Review how far you’ve come
  • Celebrate improvements and milestones
  • Gently reassess key movements, balance, or strength if needed
  • Adjust your plan if your situation or confidence has changed

  • Offer advice on posture, joint care, or using mobility aids
  • Signpost to community classes, local exercise groups, or additional services if appropriate

Your physiotherapist is there to encourage, motivate, and ensure you feel confident between visits.

Before leaving, your physiotherapist will:

  • Summarise what you have worked on
  • Agree what to practice before the next session
  • Confirm when they’ll see you again
  • Make sure you have all the information and equipment you need

They’ll leave you feeling supportive and positive about your progress.

Your follow-up sessions are about empowerment and partnership – not just exercise.

Your physiotherapist’s goal is to help you:

  • Feel safe and confident moving at home and outdoors
  • Regain or maintain your independence
  • Stay active and enjoy the things that matter to you

Every session is shaped around you – your comfort, you pace, and your goals.
